About Us
Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all the primary and specialty health care needs of adults and families in the heart of Central Brooklyn. Each year, BSFHC provides more than 57,000 patient care visits to over 15,000 unique patients, who reside in 50 different ZIP codes across the five boroughs of New York City. BSFHC delivers care at five sites: our medical headquarters on Brooklyn’s Fulton Street, our satellite Broadway Family Health Center, and our three School-Based Health Centers.

To become the model of excellence for community-based primary care, and to act as a catalyst to promote health, prevent disease and address socioeconomic factors that adversely affect health, as an integral member of the community.
Communities Served & Target Populations:
BSFHC target health services to the neediest among the 451,822 residents of our federally-defined Service Area. Fifty-seven percent of Service Area residents are Black, 20% are White, 20% are Hispanic, and 3% are Asian. Since our founding in 1978, BSFHC’s Service Area has expanded from the immediate neighborhood to include a 9.1 square-mile-wide swath of Central Brooklyn that encompasses seven Brooklyn ZIP codes: 11216 (Bed/Stuy); 11205 (Fort Greene); 11213 (Crown Heights North); 11221 (Bushwick); 11233 (Ocean Hill); 11212 (Brownsville); and 11207 (Highland Park). These medically-underserved communities were long-beset by grave health disparities and high mortality rates, substantial economic barriers and social marginalization. Despite these barriers, the communities we serve are resilient and filled with community pride.
Today, BSFHC is recognized as a provider-of-choice for delivering quality health care in Brooklyn, because we have developed and maintained strong partnerships with patients and families to help them take control of their own care to manage their conditions, achieve personal goals, and attain optimum health. BSFHC achieves the highest standards of quality for the delivery of services to patients across the lifecycle, with an interdisciplinary team of providers and staff working cooperatively to provide a full complement of preventative, primary and specialty care services, including behavioral care, as well as other supportive and enabling services.
The New Building
BSFHC’s new facility was completed at a cost of $26 million. The new health center is thus more than a new building. It represents a renewed investment in the comprehensive health care of Bedford Stuyvesant- an area that has lagged for too long behind the city and nation in the indicators of healthy living, behavioral health and wellness. In the new 38,000 square foot facility, BSFHC will be able to serve all of the primary health care needs of more than 20,000 Brooklyn residents.

On the Horizon
BSFHC is partnering with One Brooklyn Health System (OBHS) and the New York State Department of Health (NYSDOH) to open a new satellite health center and a new Specialty Care Practice, funded by a new $4.5M grant from the Dormitory Authority of New York State (DASNY). Our new sites, which are expected to open in 2020, will expand access to care for medically underserved residents of North and Central Brooklyn.

The City Council has been generous in its support for this worthy project. Others contributing to this project have included the New York State Department of Health with a pledge of $1 million, and the Primary Care Development Corporation has provided soft loans for site acquisition and supplementary design, as well as other technical assistance.
