Since 2010, BSFHC has achieved the highest level of recognition as patient-centered medical home by the National Committee for Quality Assurance (NCQA) – a strong indication of our patient-centered model.


BSFHC deemed Health Center Quality leader for its clinical performance, placing in the top 30% of the adjusted quartile rankings for clinical quality measures. This award recognizes the highest performing health centers nationwide as well as those health centers that have made significant quality improvement gains from the previous year.

Patricia Fernandez, MPS, CEO/President, welcomed the honorable guest at the gate. The CEO discussed critical health challenges faced by families in North Central Brooklyn and gave an overview of strategic plans to meet increased need for the healthcare services at BSFHC.

About BSFHC Team

1976 BSFHC was founded by Bedford-Stuyvesant Restoration Corporation
1978 BSFHC launched operations in a 3,000 square-foot facility in Brooklyn's Restoration Plaza.
1989 BSFHC relocated to a 10,000 square-foot facility to meet increased patient demand.
2006 BSFHC opens School-Based Health Centers to increase children’s access to care.
2007 BSFHC opened our 3,000 square-foot satellite site, Broadway Family Health Center
2012 BSFHC relocated to a brand-new four-story, 38,000 square-foot medical facility.
2015 BSFHC partnered with Community Care of Brooklyn, a New York State Medicaid DSRIP Performing Provider System (PPS).
2017 BSFHC and Broadway Family Health Center are recognized by the National Center for Quality Assurance (NCQA) as Level III Patient Centered Medical Homes (PCMH).
